Information For Eaton Class Members

Eaton class members have the option to participate in an Individual Health Assessment and Medical Access Program (hereafter the “Program”).  The purpose of the Program is to provide Class Members with a baseline assessment of their current cardiac condition, to formulate individual recommendations for each Class Member (if needed), to facilitate and provide Class Members with additional access to cardiac care on a scheduled basis if desired and to educate Class Members on proper cardiac health and care going forward.  Last year’s program consisted of an initial screening/evaluation visit conducted by the Program cardiologist. 


The last of three annual follow-up visits will be offered on October 12 and 13, 2020.  To schedule an appointment, please call (877) 861-1443 (toll free) during the hours of 6 am to 6 pm PST (8 am to 8 pm CST) and speak with a representative.  The scheduling/registration period for the Program closes on October 7, 2020, so you must call before that date to schedule any desired appointment.  Once you have scheduled an appointment, a confirmation and registration packet will be sent to you.  You will need to complete the registration packet and bring it with you to your appointment.  Your visit will be in Selma, Alabama on the campus of Vaughan Regional Medical Center.


Class Members have the right to elect not to receive any or all of the care available to them.
