Registration Information
Welcome to Vaughan Regional Medical Center. The Registration Department is committed to providing you with excellent customer service. We ensure that all steps will be taken to protect your privacy while you are a patient in our department. When you arrive at the hospital, please report to one of four areas for registration:
Inpatient / Outpatient Registration
The Admitting / Outpatient department is located at the Main Entrance of the hospital. All patients must stop at the front reception desk before proceeding to the Admitting / Registration office.
The hours of operation are Monday through Friday, 7:00 am to 4:30 pm. After business hours, all patients should report to ER Registration. The Inpatient/Outpatient Regstration telephone number is 334-418-4366. The fax number is 334-418-3528.
Outpatient Radiology Registration (mammograms and x-rays)
The Outpatient Radiology Department is located in the Medical Office Building adjacent to the hospital.
The hours of operation are Monday through Friday, 7:30 am - 4:00 pm. The Outpatient Radiology Registration telephone number is 334-418-4159. The fax number is 334-418-3545.
Emergency Room Registration
The ER Registration department is located inside the Emergency Room entrance of the hospital located on the West side of the hospital. All Emergency Room patients must stop at the Triage window prior to registration.

Outpatient Rehabilitation Services Registration
The Outpatient Rehabilitation Registration department is located at 1013 Medical Center Parkway.
The hours of operation are Monday through Friday from 8:00 am to 5:00 pm. The Outpatient Rehabilitation Registration telephone number is 334-872-2222. The fax number is 334-418-2232.
Registration Requirements
Upon arrival in the Registration department, you will be asked to provide the following:
- Health insurance card(s)
- Photographic identification card
- A valid physician order for all tests not scheduled
You will be required to sign the Consent at time of registration. Medicare recipients will also be asked to complete a "Secondary Payer Questionnaire" and sign an "Important Message from Medicare."
Deposits / Copayments
Patients are responsible for all financial liabilities including co-payments, deductibles and out-of-pocket expenses at the time of registration or at the time of discharge for admissions. If the amount has not been determined, you will be asked to pay the estimated amount and will be billed for the remainder. We accept all major credit cards, cash or personal check.
Pre-Registration
Pre-registration is available to all scheduled patients prior to service to expedite the check-in process. During the pre-registration process, information is obtained over the telephone. When you arrive you will be required to provide the required information and sign all necessary documents. You can pre-register by calling 334-418-4112 during normal business hours.
